- Please specify whether the correspondence address is same as or different from the residential address.
2. OnePlatform's Personal Details Update Form
If client holding more than 1 policy under OnePlatform, please be reminded to change Client Personal Details for every individual principal.

4. Address proof is not required unless it is required by YF Life, or OnePlatform has to conduct further verification with the client’s identity. (Updated: 24 Oct 2017)
