4. For Heng An Standard Life (Asia) Limited (SL), daily cut off time is HK business day before 3pm received by SL. Application and relevant documents MUST be submitted to Heng An Standard Life within 30 calendar days after sign. Please submit the completed application documents to SOD 2 working days in advance for meeting Heng An Standard Life’s deadline for processing. (Updated: 19 Sep 2017)
5. Address proof is not required unless it is required by Heng An Standard Life, or OnePlatform has to conduct further verification with the client’s identity. (Updated: 2 Nov 2018)

A. For changing of name of policy owner or life insured, please fill item 1 and submit HKID and deed poll copy
B. For changing of Residential/ Correspondence address, please fill item 2

4. All copies of original supporting documents submitted (including identification document) must be properly certified by suitable certifiers as set out in the Anti-Money Laundering and Counter-Terrorist Financing (Financial Institutions) Ordinance. The certifier must
- State that the copy document is a true copy of the original;
- Sign and date the copy document (printing his/ her name clearly in capitals underneath);
- Clearly indicate his/ her position or capacity on it.
