Important Notice
1. Most documents should be submitted to SOD within 26 calendar days after sign date of client.
2. OnePlatform's office address is NOT accept as correspondence address of client.
3. Softcopy of form is accepted, it is not required to sign on every single page and it is only required to sign the signature box.
4. Residential address proof is required if change residential address.If clients update correspondence address only, it’s unnecessary to submit the address proof.(Updated: 20 Aug 2021)

For joint named policies, please also write the names of all policy owners on the form with ALL policy owner's signatory.
