1. AIA forms and illustration should be submitted to SOD within 10 calendar days after sign date of client.

1 Request for Investment-linked / Universal Life Plan Services ([R11])

  • In Part A Q2, be reminded that the "Investment Allocation" total MUST be 100%.

Non-HK Permanent Resident (Additional Documents)

1. AIA HK shall require the following identity documents of the applicant who holds a HKID card but has not obtained the Hong Kong Permanent Resident status. (Client have to re-submit ID if no valid Copy of identification document in captioned policy)

  • HKID card copy and,
  • A valid travel document (e.g. an unexpired international passport, Exit-entry Permit for travelling to and from Hong Kong and Macau for Official Purposes, etc). or
  • A relevant national identity card bearing the individual's photograph; or
  • Any government or state-issued document which certifies nationality.

For Overseas Client - Exclude PRC Client / Macau ID Holder (Additional Documents)

PRC ID Holder (All documents must be ready before "認證" or "i-Verify")

3) Landing slip issued by Immigration Dept.

For age 0 – 6

Special Case

1. If your client chooses to pay quarterly, semi-annually or annually, the 1st payment can be paid by Cheque issued by the client payable to AIA International Limited should be attached. If the cheque is in HK currency, please check the exchange rate from AIA's website. (updated: 23 April 2014)

2. AIA Financial Health Check Form is needed if the basic insurance plan is :

  • Annualized premium is HK$20,000 or above; OR
  • Single premium is HK$200,000 or above; OR
  • Sum insured is HK$2,000,000 or above.
  • If in other currency, based on exchange rates at the date of application is completed.
  • If client is not willing to complete this form, he/she is required to declare on the "Remarks" of the form or on "Special Request" with client’s signature. (updated: 23 April 2014)
