Important Notice
- Third party payment for policy premium is not encouraged. In occasion when third party payment is necessary, Payors are required to complete the Third Party Payment Declaration Form ([T1]) and provide his / her identity card plus valid relationship proof (e.g. government-issued marriage certificate, birth certificate, etc.) with Proposed Policyowner / Proposed Insured
- Payor’s identity (e.g. full name) will be verified and his / her relationship with Proposed Policyowner / Proposed Insured will be confirmed by the provided valid relationship proof before proceeding premium transaction.
- In special circumstances that the payment is not made by Proposed Policyowner / Proposed Insured, Fubon Life only accepts payment from the following relationships with Proposed Policyowner / Proposed Insured:
- Spouse
- Parent
- Child
- Local company owned by proposed policy owner / policy owner with at least 25% shareholding
4. Fubon Life should refund the unidentified fund or for payment received from person other than above mentioned via original payment channel directly.

- Certified true copy of the proof of relationship between the third party and the policy owner (e.g. marriage certificate, birth certificate).

Checklist for Employer Company Pays for Key Person:
- Business Registration Certificate / Certificate of Incorporation and latest Annual Return for company entity

* If Proposed Policy Owner / Policy Owner owns less than 50% shareholding, signature of another authorized signer is required.
