2.Certified true copy of Certificate of birth: to prove the relationship between the payer and the policy owner
3.Certified Address Proof of the third party payer

Certified with consultant printed full name, signature date of certification and company stamp

Chinese document needs English translation
4.Certified HKID copy of the third party payer With signature of consultant, printed full name of consultant and date of certification and company stamp.
Husband pays for wife (as wife as policy owner) / wife pays for husband (as husband as policy owner)

Company pays for director (For Director who owned the company, and the Director is the policy owner)
1. Letter to explain the relationship between the payer and the policyholder ([L5])

the reason why the company is paying the premium for the policyholder.

To be signed by either HR Department or the Director of the company
2. A certified copy of the Certificate of Incorporation and the Business Registration Certificate
3. An update shareholder lists to show that the policyholder is the shareholder of the company
4.Source of Wealth information

Nature of company & amount of annual profit.

If the case is above FPI SOW limit, (please refer to Source of Wealth table), will require a certified copy of the latest audited company accounts
