- All required documents must be original or certified true copy. For all the certified true documents, please have them to be certified by suitable certifiers (e.g. insurance broker, lawyer, notary public or professional accountant) and please be reminded to include the wording "I confirm that this is a true copy of the original document" with the following information:
- Name of Certifier
- Signature of Certifier
- Company Name
- Date of Certification
- Address proof is not required unless it is required by Transamerica, or OnePlatform has to conduct further verification with the client’s identity. (Updated: 7 Feb 2018)
- Receiver needs to bear the necessary bank charge (if any) incurred for the transferred funds
- Please note that an additional document(s) may be required subject to PLA’s review.

- Certified true copy of identity document of Policy Owner. For non-Hong Kong/Macau permanent resident, nationality proof/travel documents copy (e.g. passport copy) is also required.
- Self-certification Forms (Individual) ([S1]) / Self-certification Forms (Entity and Controlling Persons) ([S2]) - Required for UL, Whole Life, ROP product
