Important Notes:

  1. Policy Owner (and all relevant parties) is required to sign the required form and submit within 1 month to Sun Life. (Updated: 24 Oct 2019)
  2. Irrevocable beneficiary is also required to sign if there is any.
  3. If the policy has been assigned as security, signature of the assignee is required.

Checklist

  1. CS eform
  2. Surrender Form ([S6])
  3. Certified true copy of identity document of Policy Owner. For non-Hong Kong/Macau permanent resident, nationality proof/travel documents copy (e.g. passport copy) is also required.
