Important Notice
- All the form details must be filled in ENGLISH.
- The hardcopy of application form should be submitted to SOD within 10 calendar days after sign date of client.
- Any request for transferring of the policy surrender value from an existing policy to pay the initial premium of a new application will NOT be accepted.
- Make sure the client's correspondence address is match with AIA's record.
- Policy document is NOT necessary.
- For ILAS, if the client applies for this option within initial contribution period, commission will be clawed back proportionally and surrender charge will be applied to client account.
- For ILAS, if the client applies for this option after initial contribution period, no commission will be clawed back but surrender charge will be applied to client account.
- Please read the Requirement for Signature of Witness
- Policy number must be filled on all AIA POS Request for Change Form, otherwise, AIA will not process the request and will return the request form. (Updated: 15 Mar 2017)

3. Certified true copy of identity document of Policy Owner. For non-Hong Kong/Macau permanent resident, nationality proof/travel documents copy (e.g. passport copy) is also required. (if AIA has not received the latest copy and/or is required by AIA)
4. Address proof is not required unless it is required by AIA, or OnePlatform has to conduct further verification with the client's identity. For PRC clients, according to the MCV guidelines, if customers declare their correspondence address is in Hong Kong, address proof is required. (Updated: 24 Oct 2017)
Payment Methods
Crossed Cheque
AIA will issue USD cheque for all payments to clients which needs around 3 weeks for clearance. Therefore, IFA Business Department of AIA will exchange into HKD cheque for all HK clients and will send the cheque to IFA direct mailing to client.
