Important Notice

1.  Please refer to YF Life Policy Administration Manual item 3.6 and 3.7 for details and updated requirement. (updated: 3 June 2016)

2.  All signed document should be returned within 5 working days from the sign date (Updated: 07 Dec 2022)

3.  Client may apply reinstatement within 12 months from the last premium due date (updated: 27 April 2011)

4.  For redating, the due date of last unpaid premium must not exceed 3 years from the policy effective date (updated: 27 April 2011)

5.  Each policy is allowed to exercise reinstatement by redating ONCE (updated: 27 April 2011)

6.  Redating is NOT applicable to all plans (updated: 27 April 2011)

Reinstate from lapsed 

Checklist

1.  Request for Policy Change Form (A01B) ([R3])

  • If any content on the form is being amended/revised, the proposed policy owner is required to counter-sign close to the revised part as confirmation;
  • If there is more than one type of handwriting on the form (such as part of handwritten content and part of computer-printed content), the proposed policy owner is required to sign on the lower right corner of each page of the form as confirmation.
  • Please complete Part II: Statement of Insurability.

2.  Copy of identity document with consultant’s declaration and sign (if a copy of valid identity document has not been provided before) (updated: 7 Dec 2018)

3.  DDA Form (For Monthly payment and Quarterly payment case) (updated: 27 April 2011)

4. Payment Method

a) For Pay back all Overdue Premiums: All overdue premiums and at least 2 monthly premium / 1 modal of planned premium ; OR

b) For Redating: At least 2 or 3 monthly premiums / 1 modal of planned premium

5.  Policy Cancellation / Reduce Premium / Reinstatement Form (CS eform)

Reinstate from lapsed (PRC) (Updated: 30 Aug 2016)

  • In cases where the application date for reinstatement is not more than six months from the Paid To Date, the policyowner is not required to undergo the procedures for verification of travel documents
  • In cases where the application date for reinstatement exceeds six months from the Paid To Date, the policyowner is required to undergo the procedures for verification of travel documents.

Simplified Reinstatement from lapsed (Updated: 21 Aug 2014)

Simplified reinstatement will be performed if the overdue premiums are submitted within 2 months from the premium due date of the lapsed policy.

All simple reinstatement applications must be made by customers either in writing or through YFLink. (Updated: 19 Mar 2025)

For electronic simplified reinstatement applications:

- Consultants can initiate the request on SMART, and customers can confirm and submit through

YFLink; or

- Customers can apply directly through YFLink.

Resume payment from premium holiday 

Checklist                                                   

1.  Request for Change of Policy Value Arrangement Form (A15B) ([R2]) refer to Item 3

  • For normal case, regular premium is settled by autopay, client needs to reserve at least 2 months premium.
  • For early bill case, clients have to submit the due premium to activate the autopay.

2.  Policy Cancellation / Reduce Premium / Reinstatement Form (CS eform)

Payment method

DDA must be submitted for Monthly payment and Quarterly payment.  (updated: 27 April 2011)

For Semi-Annual and Annual payment, client can pay the premium by cheque.  (updated: 27 April 2011)
