Reinstatement - Utmost Worldwide

Quilter International policies do not apply to these forms, please get it via Service Log to principal.

Important Notice

1.   Reinstatement is only allowed to all outstanding premium / contribution being paid within ICP / surrender value less than 1,500.

2.   If the policy is held in trust, then the settler of the trust should sign the form.

3.   If the ICP is not completed, all the outstanding premiums have to be paid, and bring the policy up to date.

4.   If the policy paid up more than 1 year, Utmost will not consider reinstating the policy.

Checklist 

For the Plans have not been contributed for LESS than 12 months

1. Managing Arrears on your Vision Plan ([M1])

       Client can choose one of the below boxes for reinstatement:

  • "Payment of Arrears" - collect all outstanding premiums on next collection date   OR
  • "Cancellation of Arrears" -  cancel the outstanding premium and collect future premiums   
  • If the client wish to pay the future premium by BSO, please write down the instruction on the request form
  • Signed Plan Tracker by the client may be required if client would like to cancel a large number of arrears or reduce the regular premium level (Updated: 15 Jul 2019)

3. If there is outstanding payment, please submit cheque made payable to "Utmost Worldwide Limited".  Payment must be made in plan currency.   (updated: 19 April, 2011)

6. International Tax Compliance (ITC) Form - Individuals ( [I5]) or International Tax Compliance (ITC) Form - Entities ([I6])

      - For HK residents to fill in TIN number, kindly refer to the 稅務編號on tax bill, which is a 10-digit number (Updated: 18 Jan 2016) [Sample of TIN Number], if client could not provide the TIN number, please fill with HKID number and state the reason. (updated: 21 Jan, 2016)

For the Plans have not been contributed for OVER 12 months (Additional Documents) (Updated: 15 Jul 2019)

1. Certified / notarized copy of Proof of Identity (photo ID/ passport/s) – if the client is residing in Hong Kong, a certified copy of client’s HKID is also required

2. Certified / notarized copy of Proof of Address (e.g. Utility bill or bank statement issued within the last 3 months. E-Statements are not acceptable)

3. Source of Funds Questionnaire ([S1])

4. Updated method of payment for future premiums (either by Credit Card or set up a Bank Standing Order)

Remarks:

(1) Client may settle all missing premiums by setting up an Arrears Collection Schedule or by applying Premium Holiday, given that the Surrender Value is above the minimum requirement (USD3,000 / GBP2,000 / HKD24,990 / EUR2,700 / JPY399,990). Maximum duration of each Premium Holiday is 12 months. In accordance with the Terms and Conditions, multiple holidays can be requested throughout the life of a Plan but are subject to Head Office’s approval on a case-by-case basis.

(2) Specimen signature form ([S3]) may requested if client’s signature does not match with the original application.

(3) A clear and legible certified copy of client’s identity is required, details must include a photograph of high enough quality that facial features can be distinguished, the document number, expiry date and MRZ line must also be clearly visible. Where the passport has been endorsed or marked as "Refer to page XX" the relevant page must also be correctly certified and submitted.

Payment Method

Credit Card

  • No extra charge by Utmost
  • Fill in Payment Card Mandate Form ([C1]) 
  • Specify the month of contribution starting to pay by credit card at the top right hand corner. 
  • The starting date should be at least 3 weeks later from the submission date.

Bank Standing Order (BSO) 

  • No extra charge by Utmost
  • Fill in Payment Instruction Form ([P2]) 
  • Please note that the following banks should use their own BSO forms: 
  1. Bank of China
  2. Bank of Communication
  3. Bank of East Asia
  4. Chong Hing Bank
  5. Hang Seng Bank
  6. Wing Lung Bank
  7. Standard Chartered Bank

For other banks, client could use Utmost BSO form.  Utmost will normally submit any original payment form to the client's bank for processing.  However, the applicant(s) should confirm the above with their bank as this is beyond Utmost control.  (updated: 21 April 2013)  

Payment Method for oversea client

Please be advised that all clients who live outside of Hong Kong should send money to Lloyds Bank with details in the Payment Instruction Form ([P2]) with immediate effect.  We will NOT be able to accept payments from non-HK residents who send money through their Hong Kong local bank accounts (updated:  28 April, 2011)
