Important Notice
1. CTF Life would arrange special handling to reinstate the Columbus policy if the policy is terminated with ICP due to non-payment laspse currently.
2. Submit the reinstatement request within 1 year of the policy lapse from termination date. (Updated: 11 Jun 2013)
3. For Cheers Investment Life Insurance Plan/Cheers Plus, if this Policy terminates automatically because the Account Value falls to zero through the operation of the Contribution Interruption provision or because of the non-payment of any Planned Premium, client may reinstate the policy subject to the following conditions: (Updated: 24 Oct 2014)
- that request must be made by client within two years after the termination date; and
- client must provide satisfactory evidence to CTF Life that the Insured is still insurable; and
- any unpaid Planned Premiums must be paid in full with interest as determined by CTF Life; and
- Overdue interest (if the initial overdue premium is overdue for more than 90 days) (Updated: 12 Oct 2015)

4. Form should be submitted within 26 calendar days to OnePlatform.

Resume contribution from Premium Holiday
Policy owner may resume Regular Premium during or after the premium holiday by
1. Submit "Request Form for Suspension Resumption of Premium Payment" ([R1])
2. Direct Debit Authorization ([D3]) and payments in advance of two months’ premiums must be submitted for policies in monthly payment mode (without a valid Direct Debit Authorization).

Reinstatement from a lapsed policy
1. For Columbus: Supplementary Statement Form for all Policy Services (only applicable after policy issuance) ([S9]) to state the reinstatement request and declare there is no health changes since policy lapsed.
For Cheers Plus: Change Form (with Health Questionnaire) ([C6])
For Oscar: Policy Reinstatement Form - Oscar ([P6]) Please note below for Oscar Reinstatement:
(1) If the policy contains one or more riders, please complete "Part II - Evidence of Insurability" of the "Change Form (with Health Questionnaire)" for assessment;
(2) Reinstatement is conditional upon payment of all items set out below:
- Total premium and premium levy from the date of termination of the policy to the date of reinstatement as if the policy had not terminated;
- Premiums and premium levy falling due within two months from the date of reinstatement;
- The surrender value (if any) paid by the Company to policy owner at times of termination of the policy;
2. Submit all outstanding premium up to current month.
