1. Reinstatement is only allowed to All outstanding premium / contribution being paid
2. Lapsed case means client has not settled premium in 45 days after premium due date.
3. Reinstatement can be applied within 2 years after date of lapsed.
4. The submission of document must be within 1 month from the sign date.
Checklist
Resume contribution from contribution holiday
1. If client would like to resume contribution during premium holiday, please advise the client to submit
"Request for Fund Transaction Application" and " tick " the box of "Cease Premium Holiday " on part 2.

1. Certified true copy of the passport / visa
To show the entry /exist date into / leave Hong Kong with Hong Kong entry stamp. The pages of client’s signature and photograph in the passport are also required. Mainland visitors should be submitted with WHOLE passport copy.
2. Certified true copy of the proof of traveling, such as boarding pass, air ticket, ship / train ticket, is highly recommended but not mandatory.
