MAINLAND CHINESE VISITOR (MCV) VERIFICATION
A. Service Scope:
- Ÿ entry proof verification (驗證), including verification at:
- Fubon Life offices (Cityplaza 4 / Dorset House)
- OnePortal offices (Fortress Hill / Cityplaza 3)
- Ÿ Visa Card or MasterCard Credit Card payment
- Ÿ application documents screening (if full set of Application documents are available)
B. Making Appointment
- Ÿ Appointment can be made by completing the "Appointment Form – Mainland Chinese Visitors (MCV) Verification Service ([A1])" and send to Fubon via Email during Service Hours.
- Ÿ Appointment requests received during Service Hours (Cut-Off Time on Mon-Fri: 17:30 / Sat: 13:00) will be replied on the same day. Requests made after Cut-Off Time will be replied in the morning of the next working day.
- Ÿ An appointment number will be assigned and communicated to insurance intermediary via Email once the appointment is confirmed. For enquiries, please contact broker hotline (2516 0196).

2. Service Hours:

Saturday: 09:00 – 13:00
3. Special Service Hours:
At least 3 working days prior to Appointment Date is required and subject to resources availability.
Monday to Friday : 17:30 – 20:00
Saturday* : 13:00 – 17:30
Sunday & Public Holiday* : 09:00 – 17:30
Note:
Blackout period - January 1 and the first 3 days of Chinese New Year (初一至初三).
*: Mobile service will be provided in broker’s office for application(s) with minimum total ANP of USD62K or HKD500K per appointment.
4. Cancellation of Appointment
For cancellation, notice must be made at least 1 working day before the Appointment Date via email.
C. Documents Required and Procedures of Entry Proof Verification (驗證):
- Ÿ Original and certified true copy (by Insurance Intermediary & Fubon Life staff) of the proposed policyowner’s and the proposed insured ’s identity, travel document & entry proof:
i) valid travel document (including pages with personal information & photos, entry date & duration permitted to remain);
ii) landing slip (with entry date & duration permitted to remain);
iii) People’s Republic of China (PRC) ID card
iv) Birth certificate of the insured child (if applicable).
v) Vaccination record (within 6 months) or student book (within 6 months) of the insured child (if applicable)
- Ÿ The Declaration for Insurance Application Verification Form ("Declaration Form") is to be completed, signed with sign date by Fubon Life staff, the Insurance Intermediary, the proposed policyowner and the proposed insured (if aged 18 or above).
- Ÿ If full set of application documents are available, Fubon Life staff would screen the documents to ensure completeness. If Insurance Intermediary does not submit the screened documents on the spot:
i) the screened documents would be returned to Insurance Intermediary with a coversheet signed by Fubon Life staff
ii) the Insurance Intermediary must return the duly completed Life Insurance Application Form, Standard Illustration, Financial Needs Analysis (FNA), Important Facts Statement – Purchase of Long Term Insurance Individual Policy (IFS-MP) , Self-certification Form and other relevant documents through their broker (withbroker’s chop) to Fubon Life within 7 working days
- Ÿ The signatures and sign date of aforesaid documents must be the same as that of the Declaration Form.
Payment by Credit Card
- General Guideline:
- Refer to Product Specification or section "Payment Method Summary" for whether card payment is accepted.
- Administration charge 1.4% on premium paid amount will be incurred and born by payor if the VISA / Master Card payment is not acceptable for the product. Please contact Fubon Broker Hotline beforehand for inquiry.
- "Request for Payment by Visa Card / MasterCard Credit Card Form ([R3])" is required for Visa Card or MasterCard Credit Card payment by credit card machines at Fubon Life Office (i.e. Card Present).
- "Request for Payment by Visa Card / MasterCard Credit Card Form ([R3])" is required, and "Credit Card Payment Authorization Form ([C1])" are required for Visa Card or MasterCard Credit Card payment by card not present.
- Card payments must be made in HKD.
- If policy currency is USD, the card payment will be paid in HKD. The current declared exchange rate is 7.8 and is subject to review and change from time to time by Fubon Life.
- Cardholder should be the Proposed Policyowner or Proposed Insured.
- It is not allowed to separate the premium payment into different transactions by using same card or different cards, unless due to the credit limit of the card.
- Card should bear with cardholder’s signature. Insurance intermediary should request cardholder to sign the card if the signature field is blank.
- Only accept VISA and MasterCard cards. UnionPay, American Express and Diners Club cards are not accepted.
