Quilter International policies do not apply to these forms, please get it via Service Log to principal.

Note: Initial premium / payment is NOT mandatory. The principal will issue pending for reminding of premium settlement after underwriting decision.

For Lump Sum payment policy, only Credit Card, Telegraphic Transfer (TT), Personal cheque and Bank Draft are acceptable.

  • Please provide payment proof (e.g. transfer advice/ TT receipt) and state policy owner’s English name and policy number clearly to Utmost for locating the payment.
  • Please note that HK client who hold HKD/USD policy should not use the Payment Instruction Form ([P2]) as it is not accepted by Utmost.

Payment Methods for oversea client who hold USD currency policy

Please be advised that client who hold USD currency policy and live outside Hong Kong should send money to Citibank N.A. New York with details in the Payment Instruction Form ([P2]). 

Payment Methods for oversea client who hold HKD currency policy

All client, no matter lives in HK or outside HK and hold HKD currency policy should transfer the payment to Standard Chartered Bank (Hong Kong) Ltd. For details, please refer to the above table.

3. Telegraphic Transfer (TT)

  • For quarterly, half yearly or yearly contribution only and lump sum payment policy

4. Personal Cheque

  • For quarterly, half yearly or yearly contribution only and lump sum payment policy
  • Please make cheques payable to "Utmost Worldwide Limited" in English.
  • Please submit client's written confirmation (letter) of his/her future payment arrangement.
  • Utmost do not accept cheque for Initial payment of monthly contribution.

5. Bank Draft

  • For quarterly, half yearly or yearly contribution only and lump sum payment policy.
  • Please make Bank Draft payable to "Utmost Worldwide Limited" in English.
  • Please submit client's written confirmation (letter) of his/her future payment arrangement.
  • Utmost do not accept bank draft for Initial payment of monthly contribution.
  • If client submit bank draft for first payment, s/he should submit a certified true copy of the bank receipt which showing money transferred from client's account.

Third Party Payment

Payments made by Spouse

Utmost will consider application where payment for a policy is made by the applicant's spouse provided the following additional information is provided:

1. Certified true copy of the paying Spouse's Photo ID/Passport

Where a client is a non-HK permanent resident i.e. they have a standard HK ID card and not a permanent one (clearly seen by the words "permanent HK Resident" on the front) – we require a certified true copy of their primary travel document (passport) as well as their HKID card.  (need the page with all the personal details and photo) (updated; 28 Mar 2012)

2. Source of Funds Questionnaire completed on the Payer together with an original Utility Bill confirming residential address 

3. Confirmation of the Payer's occupation details 

4. Written confirmation from the Payer confirming their relationship with the applicant and that they understand that they have no rights to the policy proceeds 

5. Where the spouses use different Last Names, Utmost will require a certified copy of their marriage certificate

Payments made by an Employer

Utmost will consider application where payment for a policy is made by the applicant's Employer provided the following additional information is provided:

1.  For Applicants whose employer is quoted on a recognized Stock Exchange the following is required:

  • A signed confirmation from the employer as to the purpose of paying the premiums for the applicant's policy and that they have no interest in the policy proceeds.

2.  For Applicants whose employer is not quoted on a recognized stock exchange the following is required:

  • Certified copy of the company's Certificate of Incorporation
  • A Certified copy of the passport with photograph or other acceptable identification papers with photograph of all the directors/officers  
  • Names and residential address of all directors
  • A copy of the Authorized Signatory list
  • A signed Director's statement as to the nature of the company's business
