  1. All forms must arrive Pacific Life within 14 working days (deadline) from application sign date and at least two working days before the deadline to OnePlatform.
  2. Client must use the same signature on Application Form and for all medical examination documents.
  3. Only Always Term and Cancer Free Protection are available for PRC clients.
  4. For application in US$, the exchange rate will be fixed at US$1.00 to HK$7.80 if payment of initial premium is in HK$.
  5. The checklist is for reference only. Further requirements or evidence may be needed subject to Underwriting.
  6. All samples provided in OnePortalare for REFERENCE only, consultant must fill in all forms with client’s latest information provided. The reference products recommended should be on OnePlatform’s shelf by the time you make the recommendation.
