Important Notice :

  1. All the form details must be filled in ENGLISH. Chinese is acceptable for the parts of residential address and employer information.(Updated: 16 Nov 2021)
  2. Zurich only accepts application of Hong Kong residents from OnePlatform.
  3. Application will be automatically cancelled once it has been pending for the outstanding requirement for over 90 days from the application receipt date.
  4. All NB application of Hong Kong residents should be submitted to SOD within 25 calendar days after signed.All NB application of PRC residents should be submitted to SOD within 5 calendar days after signed. (Updated: 27 Feb 2023)
  5. All samples provided in OnePortal are for reference only. Consultant must fill in all forms with client's latest information provided. The reference products recommended should be on OnePlatform's shelf by the time you make the recommendation. No same service's products should be recommended unless there are no other suitable products from other providers.

Identity and entry proof verification for mainland China residents

1. ZOOM eSubmission

New business application, identity document and entry permit can be conducted via ZOOM as e-verification.

2. Paper submission For MCV customers: all copies of identity and entry permit of proposer and proposed insured must be verified and certified true copy with date by the licensed insurance intermediary and our Distribution Support Team in our office.
