Important Notice  

1.  HK Local Case: Application form and relevant documents MUST be submitted to SOD within 30 days after the Proposed Policy Owner(s) sign(s) the form.

2.  Non-HK residence Case: Application form and relevant documents MUST be submitted to Heng An Standard Life (SL) within 90 days after the Proposed Policy Owner(s) sign(s) the form. For Mainlander residing in China, Application form and relevant documents MUST be submitted to Heng An Standard Life (SL) within 7 business days.

3.  All copies of original supporting documents submitted to SL must be properly certified by suitable certifiers as set out in the Anti-Money Laundering and Counter- Terrorist Financing (Financial Institutions) Ordinance (i.e. authorized HK insurance broker, notary public). An acceptable certification should have the following features:

The copy document is signed and dated by the suitable certifier

The certification statement should state that it is a true copy of the original (or words to similar effect)

The certifier has clearly indicated his/her position or capacity on it

The name of the person providing the certification must be clearly and legibly printed

4.  Not available to U.S. and Canadian residence

5.  For Life Insured who is not the policy owner does not need to sign on Application Form for ILAS products

6.  Under Question 1 in Part I of the Important Facts Statement and Applicant’s Declarations (“IFS & AD”) , the following is one of the purposes accepted by SL:

“Investment with minor life protection”

7.  For Heng An Standard Life (Asia) Limited (SL), daily cut off  time is HK business day before 3pm received by SL.

8. All samples provided in OnePortal are for REFERENCE only, consultant must fill in all forms with client’s latest information provided. The reference products recommended should be on OnePlatform’s shelf by the time you make the recommendation. No same provider’s products should be recommended unless there are no other suitable products from other providers. (Updated: 19 Nov 2019)

2.     Identity Card or Passport Copy

  • For each of the Proposed Policy Owner(s), Proposed Life Insured(s), preferably beneficiary(ies) and Beneficial owner(s) (if applicable) 

 i)  HK permanent resident – HKID card copy

 ii) HK non- permanent resident – HKID card copy and valid travel document

  • For non-HK residents - A valid passport copy including the “Biodata” page of the passport (with photo, name, date of birth and passport number) and the page with entry stamp from the HK Immigration Department as valid entry proof. (Children under age 18 must also provide a birth certificate & ID copy.)

3.    Initial Contribution

If the payment currency is different from the policy currency, SL will use exchange rate to convert the client’s payment. The payment is then converted into an amount equivalent in policy currency, and converted to the unit subscription e.g. currency ‘x’ -> policy currency -> units

  • Please complete OnePlatform Financial Needs Analysis Form and "IFS & "AD" by each of the Proposed Policy Owner

5.    Risk Profile Questionnaire (Please refer to Form [R1]) - Required for application signed on or after 1 Jan 2016.

6.     Personalized Illustration

  • Signed by each of the Proposed Policy Owner(s). 
  • Sign date must be on or before the application form sign date.
  • Client's name and date of illustration cannot be amended with counter sign by client.

SL will use the client’s attained age to determine the evidence required.

Backdating is not allowed. 

Forward dating is not allowed for all policies.​​​​​​​

Source of Wealth

Additional supporting documentation of source of wealth will be required if single premium is equal to or greater than US$375,000 (HK$3,000,000); or if the regular premium is equal to or greater than US$10,000 (HK$80,000) per month / US$120,000 (HK$960,000) per year. Please note that the above rule will apply to New Business and also Top-up cases on existing policy(ies), and the combination of both.

SL would allow payment made by the policy owner(s). Payment made by third party is allowed who has insurable interest, policy owner and the third party should fill in and sign the third party payment form with the Third party ID copy.  SL reserve the right to request additional information or documentation if deemed necessary at all premium levels.

For joint owners, each policy owner should indicate the source of wealth.

Addendum/Supplement to Application

Any alteration made after the submission of the application but before policy issue, should be requested by the Proposed Policy Owner(s) using the Addendum/Supplement to Application form.

If there is insufficient space on the application form to include a full answer to a question, the full answer should be provided on the Addendum/Supplement to Application with the signature of the Proposed Policy Owner(s) and the Proposed Life Insured.

Policy Delivery

Policy will only be issued after the completion of all necessary forms and the application has been underwritten and approved by us. And the initial premium should be paid in full.

In case there is a change of insurability, the policy should be withheld and full details should be submitted to SL for reconsideration.

After policy issue, the policy pack will be delivered to the broker’s office.

Upon delivery of the policy, broker/consultant should sign and return the attached ‘Receipt Confirmation’ to Underwriting Department.

Underwriting Follow Up

SL will fax/email the Underwriting Notification to the consultant via the Broker’s company.

After the underwriting notification is faxed/ emailed out, the online Broker’s Database will also be updated.

If SL do not receive any reply from the consultant/client after 30 days from the last requirement issued date, SL will issue a final reminder to the consultant via the broker’s company.

If SL still do not receive any reply after 14 days from the issue date of the final reminder, SL will close the file and arrange refund of the initial premium.

Method of refund will be the same as the payment method on the initial premium. If refund by cheque, the cheque will be delivered to the broker/consultant’s office.
