7. Foreign Account Tax Compliance Act (FATCA) (MSP13283) ([F4])
From 1 July 2014, Zurich will be required to identify the FATCA status of all new customers during the onboarding procedures. Therefore, client must complete and sign the "FATCA form" and "Supplementary application form for corporate and trustee policies" if client submitted OLD version of new application form and the case is still pending on 1 July 2014. Private Limited Companies (Additional Documents)

ID documents (including proof of address) for at least two directors, one of which must be an executive director (please see the list under individual policy owners section for acceptable forms of ID).

Details of the corporate investor’s main bank account – the name, address, sort code, account number and account name.

ID verification of all shareholders holding 20% or more of the issued share capital as at the date of the application. Where the 20% holder is a holding company or trust, or nominee, further verification of ID of its ultimate beneficial owner(s) must also be provided.
For Futura II New Application (Additional Documents)
Please submit the following documents in addition to the individual application requirements:
1. Submit the following required documents which must be on Company letter headed stationery and signed by an authorized official(s) as per the signed mandate.
- Certificate of Incorporation or equivalent document and evidence of the registered office address (if the registered office is not the address shown on the application, please provide satisfactory evidence and details as to why this address is being used).
- A list of all directors.
- A set of the latest annual report and accounts.

For Hong Kong incorporated company
- Certified true copy of identification documents of ALL directors which can be certified by licensed consultants.
- Certified true copy of a full company search report at the Hong Kong Company Registry. (Either the latest Annual Return or similar company search report which contains information of name and identification of all directors and shareholders, and the company’s registered office address.)
For Company incorporated outside Hong Kong
- Certified true copy of identification documents of ALL directors which can be certified by licensed consultants.
- Certified true copy of a full company search report in one of the three verification methods of below:
(1) A similar company search enquiry of the registry in the place of incorporation and obtain a company report.
(2) A certificate of incumbency or equivalent issued by the company’s registered agent in the place of incorporation.
(3) A similar or comparable document to a company search report or a certificate of incumbency certified by a professional third party in the relevant jurisdiction verifying that the detail, contained in the said document, is correct and accurate.
For Private Limited Companies (Additional Documents)
