Important Notice

1.  There is administration charge pay by client's cheque of HKD200 for reissuing policy.

     Blue will send the reprint policy to client directly by register post.

2.  The submission of document must be within 1 month from the signed date

Checklist

1.    Request for Policy Services Change Form ([R7])

2.    CS eform

3.    Client's certified ID copy

      The client should also submit a copy of his/ her most updated identification document to Blue together with their policy service request unless the same has been submitted before
