Important Notice

1.   The submission of document must be within 1 month from the sign date.  (Updated 5 March 2009)

2.    Change of agent request shall be rejected if the policy is within the Initial Contribution Period (ICP) and a new

       form must be submitted after the policy’s ICP in order for the agent change to be processed.
