Important Notice

1.   Application should be submitted to SOD within 26 calendar days after sign date of client.

2.   For Vista 2, please refer to the table for minimum contribution (Vista 2)  (updated: 15 June 2009)

3.   For Vista 3, please refer to the table for minimum contribution (Vista 3)  (updated: 15 June 2009)

4.   Please read the Summary of Scenario after decrease of premium. (updated: 1 Aug 2013)

1.  If client only does decrease request, please be advised to submit below:

Payment Methods

For security reason, do not copy and send the back page of credit card to SOD/Zurich. (Updated: 6 Dec 2013)

If the client using

1.  Direct Debit

  • No need to fill Direct Debit Authorization again.

2.  Credit Card

  • No need to fill Credit Card authorization again.

3.  Standing order

  • Please submit a Standing Instruction Amendment Form directly to the bank (i.e. HSBC or Hang Seng Bank)
  • Zurich ONLY accepts Standing Order instructions from Hang Seng Bank and HSBC.
  • Please use bank’s own Standing Instruction form to execute the transaction.

Pacific ONLY:

  • Premium decrease is only applicable after the initial contribution period of 36 months (Updated: 25 Nov 2015)
