Important Notice
1. Most AIA forms should be submitted to SOD within 10 calendar days after sign date of client.
2. If the plan is NOT within Initial Contribution Period (ICP), no extra charge will be deducted from client account. The client can reduce the contribution to a minimum to USD150 per month.
3. If the plan is still within Initial Contribution Period (ICP), then, expense recovery charge will be deducted from client account and commission will be claw backed proportionally.
4. All the form details must be filled in ENGLISH.
5. AIA accepts original printed copy, photocopy or printed copy from our own printer.
6. Policy number must be filled on all AIA POS Request for Change Form, otherwise, AIA will not process the request and will return the request form. (Updated: 15 Mar 2017)
