Important Notice
1. If death certificate is not available, the claimant can provide us with supporting document for proof of death. (e.g. Cremation Certificate). Please note that the original death certificate must be furnished to us once available.
2. Upon receipt of proof of death, the company will suspend collection of premium.
3. Claim Administration will notify Consultants of further requirements after the claim has been properly reported.
4. Interview with the beneficiary / claimant may be required.
Checklist for Death Claim
1. Life Claim Application Form (C03) ([L3]), completed by the claimant
2. Original Death Certificate
3. Original Policy
4. Valid copy of identification document of insured and beneficiary/claimant/ Estate Administrator (e.g. valid copy of identity document, passport or birth certificate) (if a copy of valid identity document has not been provided before)
* If English name of beneficiary / claimant is not shown in the identity document, please provide the English name of beneficiary / claimant.
5. Proof of relationship between insured and beneficiary/claimant (e.g., copy of marriage Certificate, birth certificate)
6. Recent 3 months’ residential address proof of beneficiary / claimant (e.g. original or valid copy of an utility bill - e.g. water, electricity or gas billing / a correspondence from Government / a statement issued by an authorized institution, a licensed corporation or an authorized insurer / a mobile phone or pay TV statement.)
7. Letter of Authorization ([L4])
8. Death Inquest Report (if applicable)
9. Autopsy Report (if applicable)
10. Police Report / Statement (if applicable)
11. Newspaper Clippings (if applicable)
12. Letters of Administration
* If no named beneficiary or beneficiary is designated to the deceased’s estate, claimant should obtain Letters of Administration from the High Court before the death proceeds can be released.
* Related policy no. and our Company name should be stated in the Letters of Administration
13. U.S. Foreign Account Tax Compliance Act (FATCA) relevant documents (only applicable for the claimant who has U.S. indicia.), please refer to Claims Manual 2.4 for the details.

Payment of Claim
After completion of claim payment, Claim Administration will inform consultant to collect the following documents at Customer Service Counter:
- settlement cheque
- receipt and Indemnity
* Recipient of death proceeds is required to sign and return the Receipt and Indemnity.
- a letter to explain details of payment (if available)
