Death Claim

Documents Required The documents required for Appointed Beneficiary(ies):

  •  a medical report, paid for at claimant's expense, issued by the attending Registered Doctor; and
  •  Certificate of Life Assurance with attached endorsement (if any);
  •  evidence that the claimant is entitled to receive the payment of death benefit(e.g. birth certificate, identify card, letter of administration or probate); and
  •  evidence of the age of the Life Assured (e.g. birth certificate or identity card); and
  •  the death certificate of the Life Assured
Payment Method Claim payment will be settled by cheque

Accident Claim

Documents Required
  •   a medical report, paid at policyowner's own expense, issued by the attending Registered Doctor; and
  •   original invoice and receipts for payments in respect of the treatment undergone (disablement benefit only required copy)
  •   copy of sick leave certificate
  •   copy of policyowner and life assured's identity card (if never submit to Prudential)
Supplementary 

Document

1. If the Life Assured successfully claimed by other insurance provider or medical program provider for same incident, please send:

  •   a claim settlement letter issued by other insurance provider or a Documentary proof of reimbursement by the Life Assured’s medical program provider

2. If the Life Assured received treatment from physiotherapist and chiropractor, please send:

  •   copy of referral letter from attending Registered Doctor or Hospital
Payment Method ​ 1. Claim payment will be settled by cheque

2. Policyowner can indicate the delivery claim cheque method on Claim Form Part I Question E:

  •   By surface mail to the policyowner's correspondence address in Prudential's record; or
  •   Via consultant

Note: If no selection is made, claim cheque will be delivered via consultant

Important Note 1. Policyowner must provide written notice to Prudential within 30 days from the date of accident and provide all the required information within 90 days from the date of accident. If fail to do so, Prudential will not consider the claim. ​

2. No original documents will be returned once submitted a claim

3. For request of return certified true copies of medical receipt(s), please complete "Request For Certified True Copy Of Medical Receipt(s)" ([R2]) together with the original medical receipts.

Crisis Cover Claim

Documents Required
  •   a medical report, paid at policyowner's own expense, issued by the attending Registered Doctor; and
  •   copy of discharge summary; and
  •   copy of laboratory / x-ray / MRI / histopathological report (film is not required); and
  •   copy of policyowner and life assured’s identity card (if never submit to Prudential)
Supplementary 

Document

If major disease caused by accident, please send:

  •   copy of police report / traffic accident report / police statement / alcohol test report

Please send the following supplementary document:

  •   copy of sick leave certificate with clear diagnosis
  •   copy of referral letter from attending Registered Doctor or Hospital
  •   copy of claim settlement letter issued by other insurance provider or medical program provider for same incident
Payment Method ​ 1. Claim payment will be settled by cheque

2. Policyowner can indicate the delivery claim cheque method on Claim Form Part I Question 7:

  •   By surface mail to the policyowner's correspondence address in Prudential's record; or
  •   Via consultant

Note: If no selection is made, claim cheque will be delivered via consultant

Important Note 1. Policyowner must provide to Prudential all the required information within 180 days from the date of diagnosis major disease. If fail to do so, Prudential will not consider the claim. ​

2. No original documents will be returned once submitted a claim

Disability Claim

Documents Required
  •   a medical report, paid at policyowner's own expense, issued by the attending Registered Doctor; and
  •   copy of policyowner and life assured's identity card (if never submit to Prudential)
Payment Method ​ 1. Claim payment will be settled by cheque

2. Policyowner can indicate the delivery claim cheque method on Claim Form Part I Question 7:

  •   By surface mail to the policyowner's correspondence address in Prudential's record; or
  •   Via consultant

Note: If no selection is made, claim cheque will be delivered via consultant

Important Note Policyowner must provide to Prudential all the required information within 180 days from date of disability. If fail to do so, Prudential will not consider the claim. ​

Hospital Claim  

Documents Required
  •   a medical report, paid at policyowner's own expense, issued by the attending Registered Doctor; and
  •   original invoice and receipts for payments in respect of the treatment undergone (hospital income benefit only required copy); and
  •   copy of discharge summary; and
  •   copy of laboratory / x-ray / MRI / histopathological report (film is not required); and
  •   copy of policyowner and life assured's identity card (if never submit to Prudential)
Supplementary 

Document

If the Life Assured successfully claimed by other insurance provider or medical program provider for same incident, please send:

  •   copy of police report/ traffic accident report / police statement / alcohol test report

2. Policyowner can indicate the delivery claim cheque method on Claim Form Part I Question F:

  •   By surface mail to the policyowner's correspondence address in Prudential's record; or
  •   Via consultant

Note: If no selection is made, claim cheque will be delivered via consultant

Important Note 1. Policyowner must provide to Prudential all the required information within 90 days from the date of Hospital discharge or in case of surgical procedure or operation performed by a Registered Doctor in his/her clinic, within 90 days after such surgical procedure or operation. If fail to do so, company will not consider the claim.  ​

2. For request of return certified true copies of medical receipt(s), please complete "Request For Certified True Copy Of Medical Receipt(s)" ([R2]) together with the original medical receipts.

PRUmyhealth Prestige Medical Plan Claim  

Documents Required
  •   a medical report, paid at policyowner's own expense, issued by the attending Registered Doctor; and
  •   copy of discharge summary; and
  •   copy of laboratory / x-ray / CT scan / MRI / histopathological report (film is not required); and
  •   copy of policyowner and life assured’s identity card (if never submit to Prudential)
Supplementary

Important Note 1. Policyowner must provide to the company all the required information within 90 days from the date of hospital discharge, or in case of surgical procedure of operation performed by a Register Doctor in his/her clinic, within 90 days after such surgical procedure or operation. Id fail to do so, Prudential will not consider the claim.

2. No original documents will be returned once submitted a claim.

3. For request of return certified true copies of medical receipt(s), please complete "Request For Certified True Copy Of Medical Receipt(s)" ([R2]) together with the original medical receipts.

Deposit of Claims Payment into Premium Deposit Account (PDA)

  • Claim payment of medical claim, accident claim and PRUmy health prestige medical plan (PPM) claim could be deposited into Premium Deposit Account of specified policy to settle premium.
  • Procedure:

(1) If policyowner selects above service, please write clearly the request and policy number that for deposit claims payment into such premium deposit account ( PDA) on the claim form – Declaration & Authorization – section with policyowner endorsed signature.  OR;

(2) Enclose the completed “Supplementary Information Form (SIF) For Deposit of Claims Payment Into Premium Deposit Account (PDA)” ([S7]) together with the claim application document;

(3) Above claim payment deposit to PDA service, the request of claim payment deposit to PDA MUST be submitted together with EACH claim application [Sample]. Otherwise, Prudential would not be able to comply with the request;

(4) A payment advice will list out claims settlement amount as usual with remarks to notify such payment had been deposited into specified policy PDA as request.

Note: The above service is only applicable to same policyowner of claim policy and deposited policy.
