Important Notice
1. Once all the required evidence is received, FPI should make settlement within 5 working days
2. Documents will then be passed to FPI head office for review and please note the re-assurers MAY/MAY NOT require additional information to our requirements before they will grant the decision to make payment.
3. This will be requested upon a case by case basis and FPI will not be able to advise you further until the basic requirement is received and be reviewed.
Death claims can be complex as a lot of a factors are taken into consideration as to exactly what forms are required. e.g. product/fund value etc/whether the policy has nominated a beneficiary etc. And it will be assessed by FPI Head Office.

3. Certified ID/Passport Copy of Claimant.
4. Certified proof of address of Claimant.
5. Request for Payment on Death ([R4])
