1. The death claim form should be completed by the claimant whose age over 18
2. Form should be submitted within 26 calendar days to OnePlatform
3. With effective from 28 January 2019, CTF Life Claims Department will only return certified true copy of the original receipts and/or copy of the submitted document upon request. Please note that ‘Certified True Copy’ is accepted by insurance company in Hong Kong as an original document. (Updated: 31 Jan 2019)
Checklist for Death Claim
1. Death Claim Form
The form itself should be submitted in original, photocopy, fax and printed from printer is not accepted
2. Proof of death
- If death occurs in HK, please submit the Original Death Certificate
- If death occurs in China, please submit the Original Death Certificate (死亡証) issued by China Hospital and /or police and Original National Certificate (公証書) of the deceased
- If death occurs overseas, please submit the Original Death Certificate (English version) issued by Government of the country which is authenticated and endorsed by the embassy.
- If death occurs in Taiwan, please submit the Original Death Certificate (死亡証) issued by TW Hospital and TW ID card/ cancellation cert. of 台灣戶口 (Updated: 19 Mar 2014)
3. Original Policy(s)
Declaration regarding lost of Life Policy is required if the original policy is lost
4. Official document showing the relationship between insured and beneficiary (e.g. Marriage Certification, Birth Certificate)
5. Proof of identity of insured and beneficiary (HKID Card or passport)
- Please add "Copy" on every page and MUST be verified by advisor with date of sign, signature of advisors, name and code of advisors (PIBA no.) (Updated: 20 Feb 2014)
6. Reports relating to the circumstances and the actual cause of death (if the deceased did not die of illness)
e.g. Police report, coroner's report, newspaper clippings.
7. CS eform
8. With effect from 29 February 2024, a valid address proof issued within last 3 months must be submitted (Do not accept e-Statement). For detail requirements and example of valid address proof, please refer to the MEMO and FAQ. (Updated: 10 Jul 2024)
