Important Notice

1. The following are applicable for claims arising out of:

a. Hospital Protector Plan (HP)

b. Hospital Benefit (HB)

c. Premier Life Hospital Benefit (PLHB)      (updated: 28 Apr 2011)

d. Golden Lifestyle Hospital Benefit (GLHB)

e. Hospital Income Benefit (HI)

f. Refundable Hospital Income Plan (RP)

g. Refundable Hospital Income Benefit (RB)

2. The claims must be received by OnePlatform within 88 calendar days from the date that medical expenses were incurred.

3. Manulife will decline hospital claims as a result of:

a. Pre-existing condition which has not been disclosed to Manulife

b. Disease in the "Exclusion Clause" of the plan/rider

4. Copies of payment receipt are accepted but for HI claims only

Checklist

1. "Medical Insurance-Hospitalization and Surgical Claim Form " [M10] duly completed by both the life insured and the attending doctor of the hospital

2.  Original payment receipt issued by the hospital

3.  Original or certified true copy of Marriage Certificate (if the claimant is the spouse of the life insured

4.  Original or certified true copy of birth certificate (if the claimant is a child of the life insured)

5.  Certified true copies of claimant's ID card and 回鄉證 and 每日住院病歷紀錄 (if the hospitalization occurs in Mainland China)

6.  Other requirements requested by the Claims Committee, if any

7.  Copy of policy owner's HKID card/Passport for living claims (claimant's HKID Card/Passport copy is also required if other than the policy owner)
