Checklist for Death Claim

1. Death Claim Form (Please refer to Form [D2])
2. Death Claim - Authorization Form (Please refer to Form [D3])  (duly completed by at least one claimant or beneficiary)
3. Original Death Certificate
4. Original identification of the Deceased (e.g. HK ID Card or Birth Certificate or Passport)
5. Original identification of the claimant(s) / beneficiary(ies) (e.g. HK ID Card or Birth Certificate or Passport)
6. Original proof relationship between the Deceased and the claimant(s) / beneficiary(ies) (e.g. Birth Certificate or Marriage Certificate, etc.)
7. Letter of Administration (if it is a death claim and no beneficiary is assigned in SLA record)
8. All claimant(s) / beneficiary(ies) will have to complete the Self-Certification Form – Individual (Please refer to the Form [S5])
9. Additional documents may be requested after review of the submitted paperwork.
