Claims - Death Claims - Generali

Important notes:

  • Generali suggests to inform by phone first when the Insured was dead
  • Written proof of claim should be submitted to Generali within 60 days from the date of death of the Insured
  • Generaliwill then arrange an interview with the Claimant(s) for further claim assessment

Checklist

    1.    Death Claim Form – Part I (Updated: 31 Jan 2024) (completed by the Claimant)  

    2.   Death Claim Form – Part II (completed by the Deceased's lastattending doctor)

    3.   Original Death Certificate

    4.   Original Policy Contract

    5.    Original Deceased's Identity Card

    6.   Original Claimant(s)' Identity Card

    7.   Additional information may be required if necessary:

  • Original Trustee’s Identity Card – for Minor Beneficiary
  • Guardianship Paper – required only if the Beneficiary is a minor and no trusteeis appointed during the life time of the Insured and normally applied throughsolicitor from High Court
  • Letter of Administration and Original Executor’s Identity Card – required only if no Beneficiary is designated & normally applied through solicitor from HighCourt
  • Notary Certificate issued by the Notaries Public of the province or city where the Insured died – required only if the Insured died in Mainland China,Philippines, etc
  • Police Report
  • Coroner's Report
  • Cremation Permit
  • Medical and / or Laboratory Test Report
  • Newspaper Clipping
  • For Death Certificate or Documents not written in English/ Chinese, the claimant need to provide the valid translation
