1. Original Death Benefit – Claimant’s Statement ([D4]) complete by the Claimant.
2. Certified true copy of HKID / Passport of Life Assured.
3. Certified true copy of HKID / Passport of Policyholder (if Policyholder is not the Life Assured).
4. Certified true copy of HKID / Passport of Beneficiary and Claimant.
5. Certified true copy of Death Certificate of Life Assured.
6. Copy of documentary proof of relationship of the Life Assured / Beneficiary / Claimant.
7. Copy of the police report & newspaper clipping (if applying for the accidental death).

2. Certified true copy of Death Certificate of Life Assured notarized by China Notary Office (Only for Mainland China case).
