Important Notice
- The death certificate, ID card of insured and ID card of beneficiary are required to be verified by AIA. The beneficiary should bring the original of these documents to Customer Services Centre of AIA for verification when collecting the death claims cheque.
- Policy number must be filled on all AIA POS Request for Change Form, otherwise, AIA will not process the request and will return the request form. (Updated: 15 Mar 2017)
- If supporting documents are not yet available, submit the claim form first for notifying AIA within the allowable period.
Death Claims
- The claim form(s) together with other necessary documents are to be submitted within 90 days after the date of death of the deceased.
Checklist of Death Claims
- Application Form for Death Claim Claimant's Statement ([A8])
- Application Form for Death Claim Physician's Statement ([A7]) (Required if death occurs within 2 years after the policy is issued or reinstated)
- Copy of Death Certificate / Notarised Certificate of Death / Court Order for the presumption of death
- ID card copy of the Decease
- ID card copy of the Beneficiary
- Copy of birth certificate if the beneficiary is below 18 years old
- Original Policy or Declaration of Policy Lost ([D15])
- Police Report / Traffic Accident Report (No applicable for Natural Death)
- Post Mortem or Coroner's report (For Accidental Death / Unnatural Cause)
- Newspaper clipping (For Accidental Death / Unnatural Cause)
- Cremation Permit (may be required)
- Travel documents (may be required for death in China or overseas)
- Legal Guardianship Paper for Minor's Beneficiary (may be required)
- Surrender of HKID confirmation note (may be required)
- Permit to import human remains (For overseas death or the deceased where home country is other than Hong Kong and Macau)
- De-registration of Residence Certificate (Applicable to the deceased where home country is other than Hong Kong and Macau)
- Letters of Administration or Grant of Probate (If beneficiary is designated as "own estate")
- CS eform
Medical Claims (For SGH / SGHR / IMP / HS / HI / HB / WLHB / PHP / Health100)
- The Hospitalization Claim Form together with other necessary documents should be submitted within 90 days after the Insured's discharge date.
Checklist of Medical Claims
- Network Surgery / Medical Claim Form ([N1]) - Part 1 completed by insured and Part 2 completed by doctor
- ORIGINAL receipt and statement of charge (Not applicable for HI / HB / WLHB / PHP / Health100)
- Receipt COPY (Not applicable for SGH / SGHR / IMP / HS)
- Hospital Discharge Summary / Sick Leave Certificate with clear diagnosis (may be required)
- Return of Original Document(s) Form (If applicable, if the form is not submitted together with the original receipt / sick leave certificate, only "Certificate True Copy" of the submitted documents will be issued)
- Individual Life & Group Claims Arrangement Form (if applicable)
- Supplementary Documents (Submit the following documents will definitely make the claim process faster)
- All laboratory and pathological reports
- X-ray, CT Scan & MRI films
- Verified travel document if hospitalization in Mainland China or Overseas
- Police / Traffic Accident Report(s), if any
- Reimbursement breakdown of other insurance companies, if any (Not applicable to HI / HB / WLHB / PHP / Health100)
- Patient card copy
- Certify copy of policyholder's ID card (if not submitted previously)
- CS eform
PA (Personal Accident) Claims
- The Accident claim form(s) together with other necessary documents (if applicable) are to be submitted within 90 days after the date of loss or termination of disability.
Checklist of PA Claims
- Accident claim form ([A17])
- Sick leave certificate(s) with Diagnosis (If no exact diagnosis on receipt, e.g. government hospital receipt)
- Original medical expense / hospital receipts
- Referral letter to specialist such as physiotherapist
- AIA may require the Insured to have medical examination by in-house doctor
- Return of Original Document(s) Form (If applicable, if the form is not submitted together with the original receipt / sick leave certificate, only "Certificate True Copy" of the submitted documents will be issued)
- Individual Life & Group Claims Arrangement Form (if applicable)
- Supplementary Documents (Submit the following documents will definitely make the claim process faster)
- Compensation Breakdown from other Insurer / Party
- Physiotherapy / Occupational Report (may be required)
- Official Employer's Confirmation on Sick Leave Record (may be required)
- Laboratory, X-Ray, CT Scan, MRI Report(s) (Copy of X-Ray, CT Scan & MRI reports are preferred to film) (may be required)
- Police Report / Traffic Accident Report(s) (may be required)
