Checklist
1. Transfer of Policy Ownership Form ([T2])
- If new policy owner is PRC client, new and existing owner need to sign the form in Hong Kong with entry proof provided.
- If the new policy owner is a non-PRC resident, the form can be signed outside of Hong Kong and entry proof is not required.
2. ID copy of new policy owner. (Updated: 07 Oct 2021)
- HKID card of the new Policy owner, if not a permanent resident, passport copy is required
- For non-HK residents (non-PRC as well), accept passport or own country ID card as an identity proof.
- For PRC resident – both sides of PRC ID card
3. PRC Resident Declaration ([P5]) (If new policy owner is PRC)
4. Important Facts Statement for Mainland Policyholder ("IFS-MP") ([I6]) (If new policy owner is PRC)
5. CS eform of new policy owner.
6. Client Agreement Application Form (CA form) of new policy owner.
7. Residential address proof within recent 3 months of the new Policy Owner. (Updated: 07 Oct 2021)
