2. IFS-MP MUST sign in HK if policy issue within 1 year.
3. Form should be signed and submitted within 28 calendar days to OnePlatform.

1. Policy Ownership Change Form ([P6])
2. Self-Certification Form - Individual ([S2]) (for new policy owner)
3. ID certified true copy (for new policy owner)
4. Entry Proof (for PRC client and policy issue within 1 year)
5. Important Facts Statement for Mainland Policyholder ("IFS-MP") ([I3]) (if new owner is PRC client)
6. Relationship proof
8. CS eform (for new policy owner)
9. Client Agreement (CA form) (for new policy owner)
10. Address proof is not required unless it is required by FWD Life, or OnePlatform has to conduct further verification with the client’s identity. (Updated: 25 Oct 2017)
