2. Change of Beneficiary Form ([C3])
- Beneficiary's full name, ID No and relationships to insured must be provided.
- If the beneficiary is a charity organization /company, its full name and BR No. must be provided.
- If the beneficiary is a juvenile, trustee can be appointed. The newly designated beneficiary of juvenile policy
- The form must be signed by an adult without interest in the policy as witness. If there is no witness, company may reject the application.
- If there is an irrevocable beneficiary(ies), consent from him/them must be provided.
- Beneficiary designation is not allowed for policies issued under "Declaration of Trust"
