Important Notice
1. In the column of "HKID/Passport No." in the beneficiary section, please provide the proposed beneficiaries’ date of birth (e.g. 09 Jul 1970) and indicate "DOB" next to the date. Following the example, Standard Life strongly recommend customers to follow the format "09 Jul 1970" instead of "09/07/1970" to avoid ambiguities which may cause delay in processing the application or servicing request.
It is mandatory to provide details - (i) Name of Beneficiary (ii) Sex (iii) Relationship to (Proposed) Life Insured and (iv) Share – for the other columns in the beneficiary section. Standard Life will not proceed the nomination or change of beneficiaries if neither the ID number or date of birth of the proposed beneficiaries is provided.
2. If no nominated Beneficiary is designated on the Application Form or any information given on the
nominated Beneficiary(ies) is/are incomplete, the Death Benefit shall be payable to the Policy Owner.
