Beneficiary - Blue (Ex-Aviva)

Important Notice

1.   All the form details must be filled in ENGLISH.

2.   Insurable interest must exist between the beneficiary and Life Assured.

Beneficiary(ies) designated during new application must have one of the following relationships with the Proposed Life Assured:

  • Parent
  • Children
  • Spouse

No insurable interest is required for beneficiary change after policy issued.

3.   Please provide the beneficiary’s details including name, identification number, relationship with Life Assured and share % clearly to avoid any potential claims dispute in the future.

4.   If no beneficiary is designated, the proceeds will be payable to the Policyholder’s estate.

5.   The submission of document must be within 1 month from the sign date

3.   Client's certified ID copy

  • The client should also submit a copy of his/ her most updated identification document to Blue together with their policy service request unless the same has been submitted before.
