Important Notice
1. It should an insurable interest on the assignment request.
2. All the form details must be filled in ENGLISH.
3. Assignor is the original policy owner, and assignee is the new policy owner.
4. Life assured will remain the same after the assignment is completed. (Updated: 18 April 2011)
5. It will take 1 month for Utmost to process since assignment involved much legal issue.
6. The plan term for Assignment is the Remaining term (in terms of months) of the policy. The no. of Remaining months can be calculated by Remaining Term Calculator.
7. The submission of documents must be within 1 month from the sign date, and the Proof of Address should be dated within 3 months when principal received the documents. (Updated: 20 Apr 2017)

8. Certified HKID copy (from assignor and assignee)
Where a client is a non – HK permanent resident i.e. they have a standard HK ID card and not a permanent one (clearly seen by the words "permanent HK Resident" on the front) – we require a certified true copy of their primary travel document (passport) as well as their HKID card. 9. International Tax Compliance (ITC) Form - Individuals ([I5]) International Tax Compliance (ITC) Form - Entities ([I6]) [both assignor and assignee]
- For HK residents to fill in TIN number, kindly refer to the 稅務編號 on tax bill, which is a 10-digit number. (Updated: 18 Jan 2016) [Sample of TIN Number], if client could not provide the TIN number, please fill with HKID number and state the reason. (updated: 21 Jan 2016)
Extra Documents
1. If the policy is paid up already, you have to draw a reinstatement letter (from the assignee)
Please note that reinstatement is considered on a case by case basis. In general, policies paid up over 1 year, we will require payment of one monthly premium for each year the policy has been made paid up in order to reinstate. (Updated: 18 April 2011)
2. Check with CS whether the assigned policy has any outstanding payment. If there is outstanding payment, the assignor should prepare a cheque and made payable to "Utmost Worldwide Limited" in policy currency (pay from the assignor bank account)
