Important Notice

1.  All the form details must be filled in ENGLISH.

2.  Even the policy owner can be changed; the original life assured cannot change after the policy start.  Therefore, the life assured will not be changed even the assignment is completed.

3.  Accept assignment without insurable interest between assignee and assignor.

4. The application should be submitted within 15 calendar days (including the public holidays) to SOD after client's signature date. (Updated: 29 March 2016)

1.  CS eform [from the assignee]

2.  Deed/Release of Assignment ([D2])

Or

Deed of Absolute Assignment  ([D5])

Deed of Absolute Assignment ([D6]) (For MISO/MSIP/MIPLUS only) 

Request for Ownership Reversion in Juvenile Policy ([R4])

(Completed and signed both by the policy owner and the Assignee)

3.  Certified True Copy (CTC) of Indentification document (e.g. ID/passport/ravel document) from both assignee and policy owner (Updated: 02 Oct 2019)

4.  Risk Profile Questionnaire ([R2]) (if applicable)

5.  Address proof (This requirement will be effective for change application submitted on or after January 1, 2020.) (Updated: 20 Dec 2019)

For Overseas client

MSIP accept most of foreign nationals except Japan resident

For PRC resident, MSIP accept their application, up to HK$250,000 (if > HK$250,000, Manulife review on case by case basis)

- Client need to fill and sign in Customer Acknowledge letter for PRC client ([C3])
