1. Request For Change of Policyholder Application Form (PS-CHG02) ([R2]）
2. Certified ID copy from assignor and assignee
3. Proof document for the relationship between the insured and the new owner.
4. Important Facts Statement for Mainland Policyholder （IFS-MP）([I1]）for the new PRC owner with valid entry proof
- The sign date of IFS-MP must be prior to / the same as that of the Request for Change of Policy Ownership Transfer. The forms must be received by China Life within 7 working days from the date of its signing. (Updated: 15 Jun 2020)
