Important Notice:
1.Additional requirements if the absolute assignee (new policyowner) is a holder of PRC resident identity card
- Intermediary has to explain individual points of the Important Facts Statement for Mainland Policy Holder (IFS-MP) ([I1]) to the new policyowner.
- The new policyowner has to sign on every page of a simplified Chinese version of the IFS-MP.
- If the absolute assignment is signed within one year from policy issue date
- If new policyowner is not a direct family member (spouse, child, parent) of the original policyowner:
- the IFS-MP has to be signed in Hong Kong
- Entry proof is required and certification of entry proof and identification documents will be done by Fubon Life authorized staff
- If the new policyowner is a direct family member (spouse, child, parent) of the original policyowner:
- the IFS-MP does not need to be signed in Hong Kong
- but relationship proof is required
- The document certification requirements will follow those of new business
Checklist:
1. For individual absolute assignee (new policyowner) – certified copy of ID of assignee.
2. Life Insurance Absolute Assignment Form ([A3])
3. Self-Certification Form and W8/9 Form if new owner is a Corporate Body or if new owner is an individual with jurisdiction(s) of tax residence outside Hong Kong (please refer to New Business Forms section)
4. Information Supplement for Corporate Bodies ([I2]) if absolute assignee (new policyowner) is a corporate body
5. Important Facts Statement for Mainland Policy Holder (IFS-MP) ([I1]) (if absolute assignee (new policyowner) is a Mainlander)
6. For Corporate Body, certified copy of:
Certificate of Incorporation
Business Registration Certificate
Company search report issued within the last 6 months (to verify the information on certification of incorporation, company ownership, name of directors and the company’s memorandum and articles of association, etc.)
Memorandum & Articles of Association
Details of ownership control and structure control of the company
Register of Director and Shareholder
Authorization letter/Board Resolution for signatories handling insurance
Certified copy of ID documents for 1) beneficial owners (shareholders holding 25% (or 10% for high risk customer) or more of the voting rights or share capital, 2) directors, 3) any individual who exercises ultimate control over the management of the incorporation and/or 4) any person who is acting on behalf of the policyowner
For non-Hong Kong companies, Certificate of Incumbency (within 6 months)
For partnership, certified copy of partnership deed
For associations, clubs, societies, charities, religious bodies, institutes, mutual / co-operative and provident societies, obtain additional information such as constitution to understand the legitimate purpose of the organization.
7. Any other document that may be requested on a case-by-case basis
