5. The switch that Zurich has already received cannot be cancelled.
6. Fund allocation percentage must be integer with min. 1%
7. Non authorized funds - not accepted
8. It is required minimum holding value USD 150 ( or HKD1000) in any one fund after switching.

This form is not used for new application. If client wants to change the investment choices in the application stage, client should submit a written letter to request.

For joint named policy, both policy owners MUST sign on Fund Switching Form. Alternatively, to apply "either signature" authorization by:
a) Submitting with the "Letter of Request for Either Signature"; AND
b) Remarking on "CS eform" or remarking on "OnePlatform Fund Switching Form".
