Third Party Payment - Zurich

A limited number of third party relationships are acceptable where the policy owner is unable to make or receive a payment involving their own account.  In these circumstances, Zurich will always need a reason for the third party making the payment and additional documentation will be required as evidence.  The acceptable relationships include:

Checklist of required documents:

1.  Third Party Payment Form (HK00111) ([T1])

2.  Supplemental Document-Declaration for Data Protection for Zurich International Life Limited ([S5])

3.   CS eform

4.   Certified true copy of the proof of relationship between the third party and the policy owner (Updated: 7 Nov 2011)

5.   Certified Address Proof (within 3 months) of third party

With Consultant printed full name and signature

Address Proof Requirement

6.   Certified HKID copy of the payor

With signature of consultant, printed full name of consultant

- For customers residing in Hong Kong but not holding a permanent Hong Kong Identity Card,  both

(1) suitably certified true copies of the originals of the passport (pages bearing photo, signature and the entry chop)

If client unable to provide either one of the suitably certified true copies of the originals of the full passport and

national identification card, the client can provide the following to provie their nationalities: (Updated: 6 Jun 2012)

(a).  Original of the client's signed declaration- state the reason why the client is not able to provide national ID nor

Chinese passport (which are the primary document for verification); plus

(b).  a valid Permit for Proceeding to Hong Kong and Macao (commonly know as 'One-way Permit')

前往港澳通行証》(慣稱「單程通行證」) or

(c).  a valid Document of Identity  《簽證身份書》(commonly know as "DI") for visa purpose.

AND,

(2) Non-permanent Hong Kong Identity card bearing photograph are needed.

Company or own employer Company pays for Director (as director as policy owner)

Checklist of required documents

1.  Third Party Payment Form (HK00111) ([T1])

Original letter, on company headed paper confirming the relationship and must be signed by a senior person whose

name and position must be shown.  The customer cannot sign the letter themselves.

If the client owns his own company, he can arrange for a HR/Finance manager or fellow partner to sign the letter.

If client is a sole trader, we will require a certified copy of the report and accounts. (Updated: 7 Nov 2011)
