Decrease Premium - YF Life

Important Notice

1.  Please refer to Policy Administration Manual section 3.2 & 3.5 for details.

2.  All signed document should return to YF Life within 5 working days after sign date and return the completed form to SOD 2 working days before the deadline(Updated: 9 Jan 2018)

3.  YF Life DO NOT accept backdate decrease request, even there is missing premium in the plan.

4.  YF Life POS will issue a confirmation letter of Change in Policy Value after Decrease in Basic Sum Insured (AU4) to consultant (need to download from Report Centre of AES).  The confirmation must be signed by policy owner before POS processes the request for decrease in basic sum insured.

5.  Client should submit 1 modal new planned premium together with the request of decreasing Basic Sum Insured to avoid any misunderstanding caused on billing with existing planned premium before the request of decrease in basic sum insured approved. (Updated: 11 Feb 2014)

Please tick item 1 (New Basic Sum Insured, Change Effective Date** and Revised Planned Modal Premium)

**Back-dating the effective date of decrease in basic sum insured is not allowed. (Updated: 11 Feb 2014)

1) If any content on the form is being amended/revised, the proposed policy owner is required to counter-sign close to the revised part as confirmation;

2) If there is more than one type of handwriting on the form (such as part of handwritten content and part of computer-printed content), the proposed policy owner is required to sign on the lower right corner of each page of the form as confirmation.

2.  Payment Method

  • Payment Slip (G04) ([P1]) – Must be submitted together with payments
  • Premium Payment Declaration Form (G86) – If cash payments >= HKD 60,000 / USD 7,500 (including Cash Deposit, Cash payments at YF Cashier Counter, Bank Counter Services)
  • Please submit two months new premium payment for deposit for change

Consultant Code:

  • For PRC Policy – Consultant’s Individual TR Code
  • For HK Policy – LY455 or Consultant’s Individual TR Code

Reminder:

  • LY455 ​​​​​​​is a company code which cannot be used for PRC policy, please use the individual TR code of the servicing consultant instead.
  • If you want to apply individual TR code, please contact service log for details.
