For Name Change:

  1. Certified copy of the deed poll
  2. Certified copy of the new HKID Card
  3. Request for Personal Information Change Form ([R4])
  4. CS eform
  5. OnePlatform's Personal Details Update Form​​​​​​​

For Address Update:

  1. Request for Personal Information Change Form ([R4])
  2. Self-Certification Form – Entity/Controlling Person is needed, if change of address to a different tax jurisdiction, and W8/W9 Form if the different tax jurisdiction is the USA.
  3. CS eform
