Customer Due Diligence on Corporate Clients

LCD has issued the directive of customer due diligence on corporate clients on 01-Apr-2016. For details and its requirement, please refer to the FAQ > Customer Due Diligence on Corporate Clients.

1.  The application and premium must be submitted to Manulife before the Policy Effective Date

1.     Application For Group Life & Health Insurance Plan (for ManuCare) (Please kindly refer to Form [A1])

2.     Agent’s Statement (GLH) (Please kindly refer to Form [A2]) (Updated: 30 April 2015)

3.     Group Life & Health Insurance Plan - Employee Enrolment Form (for less than 10 EEs) (Please kindly refer to Form [G2]) /

Group Life & Health Insurance Plan - Group Employee Enrolment Sheet (for 10 EEs or more) (Please kindly refer to Form [G1])

4.     Cheque for first year premium, payable to "Manulife (International) Limited"

5.     Certified Copy of BR (Policy Holder & Affiliate Co.)

6.     Customer Supplemental Information Collection Form – Policyholder (AML Required Document) (Please kindly refer to Form [C1]) (Updated: 21 May 2014)
