Initial Premium: At least 2 Months’ Premium for monthly payment mode

Exchange Rate: - Please refer to https://www.tahoelife.com.hk/tl/tc/customer_supports/exchange_rates/ for daily exchange rate. The exchange rate will be updated on the first business day of a month.

(only 4 branches accept deposit USD cash)

2 Credit Card (Sale Slip)

* Sale Slip for initial premium and renewal premium

  • Visa and Master Card only
  • Please state policy number or broker’s code on slips
  • 1.8% credit charge is applicable
  • Credit charge will be settled by the deduction of commission
  • Issued by the banks in China credit limit is USD 5,000 per transaction
  • NOT applicable for Single Premium Policy, Prepaid Premium, Lump Sum Investment
  • Not applicable to "Brilliant Saver Protection Plan" and "Supreme Saver Protection Plan"
  • Please refer to the below part "Appointed Third Party Payor" if payor is appointed third party
3 Personal Cheque

For initial and renewal policy

  • Payable to "Tahoe Life Insurance Company Limited"/ "泰禾人壽保險有限公司"
  • Postdated cheque is not acceptable
  • Company cheque is not acceptable unless policy owner submit with the related document( i.e. annual return, BR to verify the relationship with company)
  • If no payor name listed on the cheque- please submit Premium Payment Declaration Form when the premium is under USD 300,000 / HKD 2,400,000
  • Overseas Bank Draft- Not acceptable
  • Please refer to the below part " Appointed Third Party Payor" if payor is appointed third party
4 TT

For initial and renewal policy

  • Credit: Dah Sing Bank
  • Please state policy number , full name of policy owner ,  as reference to set up the TT payment instruction
  • Please state (1) remitter name and address; (2) policy number and name of policyowner (please specify type of payment); (3) beneficiary name: "Tahoe Life Insurance Company Limited"; (4) beneficiary address: 19/F,1111 King’s Road, Taikoo Shing, Hong Kong; (Updated: 23 Aug 2021) (5) beneficiary bank name: "Dah Sing Bank, Limited"; (6) beneficiary bank address: G/F, 108 Gloucester Road, Hong Kong; (7) bank account number:
  • All bank charges are payable by policyowner, both sides’ bank charges & other related fees will be deducted from the payment
  • Please submit a copy of receipt
  • If initial premium is paid by TT but no policy no. could be provided, please submit copy of TT form & TT receipt together with application form (state clearly (1) English name of policy owner & life insured (2) Payment for Initial premium of new policy on TT Instruction form
  • Please refer to the below part " Appointed Third Party Payor" if payor is appointed third party
5 Bank Draft

For initial and renewal policy

  • If no endorsement on bank draft & payment under USD 300,000 / HKD 2,400,000,  please submit with Premium Payment Declaration Form
  • If no endorsement on bank draft & payment over USD 300,000 / HKD 2,400,000, please submit with 1. Premium Payment Declaration Form & 2. Payment Proof
  • Please refer to the below part " Appointed Third Party Payor" if payor is appointed third party
6 AutoPay (Bank Account)

For Renewal Policy Only

  • Please submit Direct Debit Authorization Form
  • Autopay credit card autopay is not applicable for "Brilliant Saver Protection Plan" and "Supreme Saver Protection Plan"
  • From 01 Apr 2019, credit card autopay application and update credit card information are not accepted, please refer to memo for details. (Updated: 22 Mar 2019)

  • Beneficiary
  • Spouse, children, brothers/sisters, grandchildren/grandparents of policyowner

(Only listed categories of third party payment will be accepted, our Company reserves the right to obtain payment/relationship proof.)

Please submit Premium Payment Declaration Form and identity proof under below conditions:

Premium Payment Declaration Form ([P1])

  • If the payor’s name cannot be shown on the deposit slip and the premium / payment is paid by the Policyowner /Insured / Beneficiary / Assignee
  • If any payment is made by Third Party’s Cash/ Local Cheque/ Credit Card/ China Union Pay Card/ Bank Account Transfer/ Phone Banking**/ Internet Banking**/ Telegraph Transfer or purchase Local cashier Order.  (** Not applicable for premium payment of new business application)
  • Payor and Sales Personnel should sign on the Premium Payment Declaration Form
