Important Notice

1.  HK Local Case: Application form and relevant documents MUST be submitted to SOD within 30 days after the Proposed Policy Owner(s) sign(s) the form.

2.  Non-HK residence Case: Application form and relevant documents MUST be submitted to Heng An Heng An Standard Life (SL) within 90 days after the Proposed Policy Owner(s) sign(s) the form. For Mainlander residing in China, Application form and relevant documents MUST be submitted to Heng An Heng An Standard Life (SL) within 7 business days.

3.  All copies of original supporting documents submitted to Heng An Heng An Heng An Standard Life must be properly certified by suitable certifiers as set out in the Anti-Money Laundering and Counter- Terrorist Financing (Financial Institutions) Ordinance (i.e. authorized HK insurance broker, notary public). An acceptable certification should have the following features:

  • The copy document is signed and dated by the suitable certifier
  • The certification statement should state that it is a true copy of the original (or words to similar effect)
  • The certifier has clearly indicated his/her position or capacity on it
  • The name of the person providing the certification must be clearly and legibly printed

4.  For Heng An Heng An Heng An Standard Life, daily cut off  time is HK business day before 3:00pm received by Heng An Heng An Heng An Standard Life.

5. All samples provided in OnePortal are for REFERENCE only, consultant must fill in all forms with client’s latest information provided. The reference products recommended should be on OnePlatform’s shelf by the time you make the recommendation. 2.Identity Card or Passport Copy For each of the Proposed Policy Owner(s), Proposed Life Insured(s), preferably beneficiary(ies) and Beneficial owner(s) (if applicable)

i)  HK permanent resident – HKID card copy

ii) HK non- permanent resident – HKID card copy and valid travel document

For non-HK residents - A valid passport copy including the “Biodata” page of the passport (with photo, name, date of birth and passport number) and the page with entry stamp from the HK Immigration Department as valid entry proof. (Children under age 18 must also provide a birth certificate & ID copy.)

PRC Resident and Non-HK residents (Additional Document) 

Mainland China

  • First page up to the last used page with entry stamp from HK Immigration Department as valid entry proof; OR
  • Copy of landing slip when proposed policy owner/proposed life insured

Macau

  • Copy of Declaration Form for holders of Macao SAR Permanent Resident Identity Card; OR
  • Copy of Arrival Card which is attached on the Macao Passport and copy of full set Macao Passport; OR
  • The copy of landing slip when proposed policy owner/proposed life insured using the e-channels.
“CareMore - Refundable Critical Illness Insurance Plan” is only applied to policy owner / life insured residents in Hong Kong, Macau and Mainland China.  Additional documents for entry proof showing physical presence in Hong Kong at time of application for policy owner / life insured are required as left hand side:
