Client can opt-out through the below methods:
1. Client calls FWD Life customer hotline at 2199-1000
2. Client may fax his/her request to FWD Life at 3409-8609
3. Client submits a written request saying that they would like to opt-out and state clearly the full name, policy number and client’s signature.
