Important Notice
1. Documents should be submitted to AXA within 1 month after sign date of client.
2. Ownership change to Financial Consultant is not allowed. Life Insured cannot be changed (Except for the following plans). (Updated: 05 Jan 2022)
3. State beneficiary designation (if any). For juvenile non-trust policy, the new beneficiary is either parent or legal guardian. If NO beneficiary designated, it will assign to new owner as the beneficiary.
4. For juvenile policy (non-trust), the new owner has to be parent or Legal Guardian of the Insured.
5. For juvenile policy issued with trust, please refer “Change / Termination of Trustee” at Point 4.9 about administrative guidelines and requirements.
6. Applicant's waiver of premium - death / Applicant's waiver of premium - death or disability (if any) would be deleted automatically if insured reaches age 18 and applies ownership change.
7. Policy can be assigned to a MCV client. For MCV client, please submit ID copy & Passport Photo page copy.
8. Not required to submit a new method of payment of new policy owner.

2. Absolute Assignment ([A1])
- If the answer of "Declaration for Acting on Behalf of Another Person for absolute assignment request" is “Yes”, please also submit “Supplement to Application - Declaration of Acting on Behalf of Another Person in Connection with Insurance Application / Policy Service” ([S10]) (Update: 10 Jul 2013)
- Required to submit MCV-IFS form if the Policyholder is mainlander and the application is signed on / after 1 September 2016. For details of requirement, please refer to Appendix. (Update: 5 Sep 2016)
3. Old and new owner’s ID card copy (certified) must be submitted for change of ownership request if ID card copy has not been submitted before.
4. Client Agreement (CA form) (from the assignee)
5. Assignee should complete and sign “Policy Service Application Form II" (If applicable)
- For juvenile policy - if Applicant's waiver of premium - death (WPD) / Applicant's waiver of premium - death or disability (WPDD) is applied for
- Otherwise, WPD or WPDD will be deleted automatically.
6. For Trust case, ID card copy of the person whom the new owner is acting on behalf of another person is required.
7. Provide new owner / assignee' declaration whether he/she is acting on behalf of another person in connection with the application. If yes, the personal particulars of the person being acted on behalf should be provided by the new owner/assignee.
